Enrolling in preschool means parents have forms to complete. Some of these are required state forms and some of these forms are those that will help Pioneer Preschool to keep your child safe.

​Enrollment form – To access this form, please click here STATE OF OHIO FORM. Print the form and mail it or deliver it to Pioneer Preschool. There is no link to submit the completed form directly to Pioneer Preschool.

Medical form – To access, please click here STATE OF OHIO FORM. This form must be printed, then signed and dated by the doctor and returned to Pioneer Preschool by the time school starts.
